This is an essential document, this document releases power from the parents to the child (you). The creator/grantor always retains power over its issue.
This form should be signed by the one who completed the application to register your birth, it is usually the mother but is sometimes the father.
As both named parents have co-creation rights, they also have the same joint ability to release their power to you so you can be the administrator over your own affairs.
If there is an issue with a parent refusing to sign this form then there is a default process IF you have made an attempt to request the parent to sign and they have refused. If both parents are deceased then the same default process can be used.
